Rx form (required fields)

This second step is for entering the required information for generating an Rx for treatment.

Clinical Photos

This field is where you can either drag and drop or upload the clinical photos required to submit the RX form.

IPR Preference

This dropdown menu is where you can select the IPR Preference for treatment, either Allowed, Minimum, or No IPR.

Attachment Preference

This dropdown menu is where you can select the Attachment Preference for treatment, either Allowed, Minimum, or No Attachments.

Conditions

This last field on the right is where you can select any additional patient Conditions, such as anterior crowding or spacing.

When you are finished entering the required RX information, click next and move on to Step 3.
