Rx form (additional fields)

This third step is for entering any optional, additional information that you would like us to know about the case.

Upload your scans

If you have any scans (.stl) you would like to upload and share with us, you can do this here.

Number of steps to start

By default, Orthosnap orders start with 3 steps, but you can adjust this to 5, 10, or a custom amount.

Number of impressions to start

By default, Orthosnap orders start with 0 impressions, but you can adjust this from 0-2.

Planned schedule preferences

This is where you can select your preferences for the patient schedule, either 7, 10, or 14 days.

The fields on the right are a drag and drop form for entering any additional patient conditions, such as anterior crowding or spacing.

When you are finished entering the optional RX information, click next and move on to Step 4.
